Answer for the people, places, providers and systems in your scope. Choose Yes only when the whole practice is in place. “Partly” means some elements are missing; “Not sure” means you need confirmation. If a provider handles a practice or you believe it does not apply, confirm the responsibility and evidence before claiming it is met.
